How to arrange a transfer between two clinics

A transfer between two clinics is not just a matter of booking a vehicle. The discharging facility, the receiving facility, the transport crew and the family all have to be coordinated. The quality of that coordination limits waiting, missing information and breaks in continuity of care.

Get both facilities to agree

The doctor at the discharging facility must state the reason for the transfer and the conditions required during the journey. The receiving facility must confirm that it accepts the patient, name the receiving ward and give an operational contact. An ambulance should not set off towards a clinic that has not confirmed it will receive the patient.

Define the level of transport

The organising team checks whether the patient can travel by standard patient transport or needs a medicalised ambulance. The decision depends in particular on stability, the need for monitoring, oxygen therapy, drips, equipment in use and the length of the journey. Any clinical change before departure must be reported.

Prepare the documents

The file handed over should contain what is needed for continuity of care: medical summary, current treatments, relevant test results, prescription, the patient's identity and the receiving ward's contact details. Confidential documents must be given directly to the authorised crew, never dropped into a public form.

Synchronise the timing

The departure time has to allow for preparing the patient, the availability of the bed, traffic and admission formalities. EMS confirms the journey after checking both addresses, the discharging ward, the receiving ward and the transport needs.

On the day of the transfer

Before departure, the crew identifies the patient, checks the instructions, secures the patient and collects the necessary documents. On arrival, it hands the patient over to the staff of the designated ward. The family should keep a phone available throughout the transfer.

In a life-threatening emergency or an acute deterioration, contact the competent public emergency services immediately. A request sent by form or WhatsApp does not confirm a transport booking.
